NDIS evidence organiser: your child’s reports and gaps

Free · Australia and the NDIS · By The Jay Way team

Before an NDIS access request or plan reassessment, it helps to know which reports you already have, what each one covers and what is missing. This organiser lists them in one place and keeps your own observations separate from what professionals wrote.

For access, NDIS guidance asks for recent evidence from a treating professional about your child’s disability and how it affects daily life. It also considers information from families and carers. What you need depends on what you are preparing for.

Check what you already have before paying for a new report. A gap on this list is something to ask your NDIS contact about. It doesn’t automatically mean you need another assessment.

How to use it

Fill in whatever you can. Every box is optional. Build the sheet, then change anything you like before you copy or print it.

Nothing you type leaves this page. It isn’t saved or sent to us, so copy or print your sheet before you close the tab. Leave out full names, NDIS numbers and anything you wouldn’t want passed on. Our site analytics only count that the tool was used.

Something you have seen yourself
My note, 12 October: Taylor needed help communicating a choice in the busy lunch area.

What’s missing or unclear
The report describes communication in a quiet appointment; I cannot find information about the lunchtime setting.

Your next step
